Medication Administration Form

Medication Administration forms are required for the nurse to administer ANY medication including prescription and over the counter medications. The forms must be downloaded, filled out with the child's physician and signed on the back by the parent in order to be valid. Please return the completed form along with the medication to the school nurse. A parent or guardian must drop off the medication to the nurse. A student may be permitted to self-administer medication for asthma and other potentially life threatening illnesses. A certification in writing from the child's physician must be received stating that the student is capable of and has been instructed in the proper administration of the required medication.
